Human Reliability Analysis (HRA), also known as Safety Critical Task Analysis (SCTA), Human Failure Analysis (HFA) and sometimes Human Factors Analysis (HFA), is an approached mandated by the HSE for use on Major Accident Hazard or COMAH sites to address the risk of human error leading to a loss of containment or other serious event.

When looking at HRAs, the HSE request practitioners adhere to the Chartered Institute of Ergonomics and Human Factors (CIEHF) good practice guidance. All of our work is overseen and signed off by a Chartered Member of the CIEHF which complies with the guidance, giving you the confidence that the work is of good quality and will be accepted by the HSE when submitted.
Our methodologies are based on the guidance published by the Energy Institute, although we have developed a few tricks of our own to improve the effectiveness and power of the approach.
We appreciate that undertaking HRAs on all of a site's identified Safety Critical Tasks is an expensive and time consuming activity, so we have developed policies and approaches that reduce the cost without undermining the effectiveness of an HRA.
